Deactivate account using Froedtert & MCW mobile app.

1. Login to your Froedtert & MCW app using your username and password
2. Go to Profile
3. Under Account Settings, tap Deactivate your account
4. In the following screen, tap the Deactive Account button
5. Tap Deactivate on the pop up screen to confirm.
6. Once the deactivation is successful, you will be logged out from the app.
