Frequently Asked Questions

Who can use MyChart?

MyChart is available to patients throughout the Froedtert & the Medical College of Wisconsin health network. MyChart is unavailable at clinics where the electronic medical record is not in use. For specific questions on MyChart availability, please contact the clinic where you receive care.

For which medical situations should I use MyChart?

Use MyChart for non-urgent medical situations such as prescription renewals, requesting non-urgent appointments and obtaining non-urgent medical advice.

Can I view a family member’s (my child, spouse, parent) medical information in MyChart?

You may request access to a family member’s information at the clinic. Detailed requirements for family or “proxy” access and the request forms are available in the Family (Proxy)Access Information section.

I requested medical records via MyChart, where do I find them?

When your request for medical records has been completed, you can access them under your Your Menu > My Records section > Document Center > click on Requested Records.

I am looking for immunization records, where do I find them?

When logged in to your account, or when accessing a proxy account, go to Your Menu > My Record section > Health Summary > click on the Immunization tab.

Immunizations that have been completed within the Froedtert & MCW health network will display here. If you have connected your MyChart account to another organization, a separate tab with those records will additionally appear, you will need to click that organization’s tab to view those records. To print records, simply click the printer icon in the upper right.

If you need to locate full immunization records from the state of Wisconsin, visit Public Immunization Record Access to conduct a search.

What if I don’t have an e-mail account?

If you are new to MyChart, you will not be able to access MyChart without a valid and unique e-mail account (meaning you cannot share your e-mail address with another family member). We suggest you create your own personal e-mail account before you proceed.

Is my personal health information secure?

Yes, Froedtert & the Medical College of Wisconsin take many precautions and are committed to keeping your personal health information secure. For more information, please see the Patient Rights and Responsibilities section.

What are the technical system requirements for MyChart?

  • You need a device/computer with access to the Internet
  • We recommend using an up to date web browser. Most browsers are compatible. *Internet Explorer is being phased out and may not provide the best experience.

Why doesn’t my activation code work?

For your security, your activation code automatically expires after 60 days. A code is not required to sign up for an account. If for any reason your code does not work, choose Create New Account > when asked about an activation code, choose “No, I don’t have one” to proceed in creating your account.

I cannot activate my account. Why is this happening?

You must accept the Terms and Conditions in order to complete account activation. Please ensure the checkbox is checked prior to continuing. If you disagree, you will not be able to proceed.

How do I deactivate my account?

If you would like to deactivate your Froedtert & MCW MyChart account, please complete and submit a MyChart Inactivation Form.
