If you wish to deactivate your Froedtert & MCW MyChart account, you can do so by following the steps below:
- Navigate to MyChart on www.my.froedtert.com or the Froedtert & MCW app.
- Select Menu (☰), scroll down
- Select Account Settings
- Select Deactivate account
- Confirm Deactivation
If you do not have access to your MyChart account you can deactivate your account by submitting the MyChart inactivation form below.
