How do I request Medical Records in MyChart?
Using the Froedtert & MCW mobile app:
- Download the app in the App Store or Google Play
- Log in or select Create New Account, if you haven’t already.
- Navigate to MyChart
- Select Menu (☰), scroll down under the My Record section, select Request Medical Records
- Answer the questions and select Send Request
Using MyChart via a web browser:
- Go to my.froedtert.com
- Log in or select Create New Account, if you haven’t already.
- Select Menu (☰), scroll down under the My Record section, select Request Medical Records

How do I download records that are released to MyChart?
If you requested medial records via MyChart, you can view them in your Document Center when they are released to you.
- Log in to your Froedtert & MCW account. If you are using the Froedtert & MCW mobile app, navigate to MyChart.
- Select Menu (☰) in the top left corner.
- Scroll to the My Record section
- Select Document Center
- Choose Requested Records
- Find your Formal Copy of Health Record and the date it was requested
- Select Download
- Don't have a MyChart account? Complete the Patient Request for Medical Records form linked at the bottom of this article.
- Requesting records for someone else? Complete the PHI-Authorization form at the bottom of this article.
