Sign up for MyChart
MyChart is a free, secure online tool where you can manage your health anytime, anywhere. If you have an existing MyChart account with another organization, you will need to register separately to access your Froedtert & MCW health information by following the steps outlined below.
Simple ways to sign up

Without an activation code
- Download our Froedtert & MCW app at www.froedtert.com/app
- Or visit www.register.my.froedtert.com
- Click “Create New Account”
- When asked if you have an activation code, select “No, I don’t have one”
- Follow the steps to verify your identity using your mobile phone

Did you receive an activation code via email, text, or during a visit?
- Download our Froedtert & MCW app at www.froedtert.com/app
- Or visit www.register.my.froedtert.com
- Click “Create New Account”
- When asked if you have an activation code, select “Yes, I have one”
- Follow the steps to create your account

- At check-in, just let a staff member know you’d like to sign up
- They can give you a code or help you register
Transferring medical records from another health network
In most cases, health networks can securely share your basic health information with each other without you having to do anything. This allows your doctors to see your records when they need to provide care.
However, some health networks may require your permission before sending your information to us. If that happens, you’ll need to give approval through that organization. We recommend checking with your previous hospital or clinic if you’re unsure.

How to update your insurance in MyChart
Using the Froedtert & MCW mobile app:
- Log in or create an account, if you haven’t already.
- Navigate to MyChart
- Select Menu (☰)
- Select Insurance Summary
- Select ‘Add a coverage’ or ‘Update coverage’ next to your current plan
- Enter your insurance details and submit
- Select ‘Add insurance card photos’ to upload a copy of your insurance card
Using MyChart via a web browser:
- Go to www.my.froedtert.com
- Log in or create an account, if you haven’t already.
- Select Menu (☰)
- Select Insurance Summary
- Select ‘Add a coverage’ or ‘Update coverage’ next to your current plan
- Enter your insurance details and submit
- Select ‘Add insurance card photos’ to upload a copy of your insurance card
