How do I add Power of Attorney or Living Will documents to MyChart?

To upload Power of Attorney or Living Will documents, make sure you have the files saved on your device in an electronic format (such as PDF or image).

  1. On the same device your documents are saved, log into your Froedtert & MCW MyChart account.
  2. Select the Menu icon (☰) in the top left corner.
  3. Scroll down and select Advance Care Planning.
  4. Tap Add a document.
  5. Locate the document on your device to upload.

Once the document is uploaded, it will indicate it is In Review. The Health Information Management department will review it to ensure it is complete and accurate before officially uploading it to your medical record.
