How do I request access to a patient's MyChart as the Power of Attorney or Legal Guardian?

You can request access to a patient's MyChart as the activated power of attorney or legal guardian.  In order to be a proxy you must have an active MyChart account. If you do not have a Froedtert & MCW MyChart account, first you must click Create New Account or contact us at 414-777-0900.  The POA paperwork, statement of incapacity or legal guardianship paperwork must also be on file for the patient.  You can submit documents to healthinformation@froedtert.com 

You can request proxy access as the POA/legal guardian to a patient via your MyChart account by following the instructions below.  

F&MCW App Instructions 

  1. Sign in to your F&MCW app 
  2. Press MyChart
  3. Press Menu in the top left corner, scroll to bottom
  4. Press Power of Attorney Proxy Request

Browser Instructions

  1. Sign in to www.my.froedtert.com 
  2. Press Menu in the top left corner, scroll to bottom
  3. Press Personalize
  4. Press Request access as the Power of Attorney to an Adult's record

If the Power of Attorney is not activated, the patient must sign an adult proxy form.  Click here to learn more. 

If you are the secondary POA, you must complete the Adult Proxy POA form linked at the bottom of this article.  Please submit the forms to the Health Information Management Team at healthinformation@froedtert.com
