How do I request access to a patient's MyChart as the Power of Attorney or Legal Guardian?

If you are the activated Power of Attorney or a legal guardian, you can request access to a patient’s MyChart account through proxy access.  To become a proxy, you must have an active Froedtert & MCW MyChart account even if you are not a patient. If you do not already have a Froedtert & MCW MyChart account, first you must click Create New Account or contact us at 414-777-0900.

The POA paperwork, statement of incapacity or legal guardianship paperwork must be on file for the patient before proxy access can be approved.  You can send these documents to healthinformation@froedtert.com

Once your documents are on file, you can request proxy access as the POA/legal guardian to a patient via your Froedtert & MCW account by following the instructions below.

Froedtert & MCW App Instructions

  1. Sign in to the Froedtert & MCW app.
  2. Select MyChart.
  3. Select Menu (☰) in the top left corner, scroll to bottom.
  4. Select Proxy Access Request Forms 
  5. Select Request access as the Power of Attorney to an Adult's record. 
  6. Complete the electronic form with the patient’s demographic information.
  7. When filling out the electronic form, select Full Proxy Access (medical + billing) or Limited Access (billing only).
  8. Select Submit Request.
  9. Once approved, you will receive a MyChart message notifying you that proxy access has been granted.

Browser Instructions

  1. Sign in to www.my.froedtert.com.
  2. Select Menu (☰) in the top left corner, scroll to bottom.
  3. Select Personalize.
  4. Select Request access as the Power of Attorney to an Adult's record.
  5. Complete the electronic form with the patient’s demographic information.
  6. When filling out the electronic form, select Full Proxy Access (medical + billing) or Limited Access (billing only).
  7. Select Submit Request.
  8. Once approved, you will receive a MyChart message notifying you that proxy access has been granted.

If the Power of Attorney is not activated with a statement of incapacity, the patient must sign an adult proxy form. Click here to learn more.

If you are the secondary POA, you must complete the Adult/DPOA Proxy Form linked at the bottom of this article.  Please submit the completed forms to the Health Information Management Team at healthinformation@froedtert.com.
