How do I invite a family member or friend to have access to my MyChart?

If you would like a family member, friend or caregiver to have access to your MyChart you can grant them access by following the steps below.

  1. Sign in to your Froedtert & MCW account on the app or www.my.froedtert.com
  2. If you sign in on F&MCW app, press MyChart 
  3. Press Menu
  4. Scroll down to Sharing, press Sharing Hub 
  5. Scroll to the bottom and press Manage Friends and Family Access
  6. Press +Invite an Adult Proxy
  7. Enter the information of the proxy you'd like to have access into the required fields   
  8. Press Send Invite

This will send an email invitation to the person you're granting proxy access to.  When they receive the email, they must press Accept Invitation and sign in to their Froedtert MyChart account and verify your date of birth.  If the proxy does not have a Froedtert MyChart they can Create an Account or call us at 414-777-0900.

 

You can also grant an adult proxy access to your MyChart by completing the adult proxy form below. To submit proxy forms, please send them via email to the Health Information Management Team at healthinformation@froedtert.com
