Animal owners who use a Froedtert pharmacy to fill their pet's prescriptions can request proxy access to the pet via MyChart. If you do not have a Froedtert & MCW account, first you must click Create New Account or contact us at 414-777-0900.
You can request access to your pet by following the instructions below.
- Navigate to MyChart on www.my.froedtert.com or the Froedtert & MCW app
- Select Menu (☰) in the top left corner, scroll to bottom
- Select Personalize
- Select Request access to an animal patient's record at Froedtert Pharmacy
