If you are listed as the guarantor on your child’s account, you will automatically be able to see their billing information without submitting an additional form. If you are not the guarantor, to view your child’s billing in MyChart, you must submit the appropriate proxy form requesting billing access. If you currently have proxy access but are unable to view billing, you may need to resubmit the proxy form to request full access, which includes billing. If you believe you already have billing access to your child, follow the steps below to view billing details:
Using the Froedtert & MCW mobile app:
- Sign in to the Froedtert & MCW app.
- Select MyChart.
- Stay on your profile and select the menu icon (☰) in the top left corner.
- Scroll down and select Billing.
- Find the guarantor number associated with your child’s account.
- Select View Balance Details.

- Find the guarantor number associated with your child’s account.
- Select View Balance Details.
Please note: The billing account will display the name of the guarantor, but the statement will show your child’s name as the patient.
