How do I request access to an adult patient's MyChart?

Family members, friends and caregivers can be granted access to a patient's MyChart account through proxy access. To become a proxy, you must have an active Froedtert & MCW MyChart account even if you are not a patient. If you do not already have a Froedtert & MCW account, first you must click Create New Account or contact us at 414-777-0900.

You can request access to an adult patient's MyChart by completing the Adult Proxy Form linked at the bottom of this article. Once completed, email the form to the Health Information Management team at: healthinformation@froedtert.com

When filling out the form, be sure to select the appropriate access type:

  1. Full Proxy Access (medical + billing)

  2. Limited Proxy Access (billing only)

     

If you are a Power of Attorney or Legal Guardian

If you are the patient and want to grant someone access

If you are the patient and would like to grant proxy access to someone, you may:

  1. Complete and submit the Adult Proxy Form below, or
  2. Send a proxy invitation via email by following the directions here.
