How do I invite a family member or friend to have access to my MyChart?

If you would like a family member, friend or caregiver to have access to your MyChart, you can grant them access by following the steps below. 

How to Invite Someone to Be Your Proxy (Medical Access Only)

  1. Sign in to your Froedtert & MCW account on the app or www.my.froedtert.com.
  2. If using the Froedtert & MCW app, select MyChart.
  3. Select Menu (☰).
  4. Scroll down to Sharing, select Sharing Hub.
  5. Scroll to the bottom and select Manage Friends and Family Access.
  6. Select +Invite an Adult Proxy.
  7. Enter the information of the person you'd like to have access into the required fields.
  8. Select Send Invite.

This will send an email invitation to the person you're granting proxy access to. When they receive the email, they must select Accept Invitation, then sign in to their own Froedtert & MCW account and verify your date of birth.

If the person you invited does not have a Froedtert & MCW account, they can Create an Account or call us at 414-777-0900.

Request to Grant an Adult Access to Your Record (Includes Billing Access)

  1. Sign in to your Froedtert & MCW account on the app or www.my.froedtert.com.
  2. If you sign in using the Froedtert & MCW app, select MyChart.
  3. Select Menu (☰).
  4. Scroll down to Settings, select Personalize.
  5. Select Request to grant an adult access to your record.
  6. Complete the electronic form with the demographic information of the person you would like to grant access to.
  7. When filling out the electronic form, select Full Proxy Access (medical + billing) or Limited Access (billing only).
  8. Select Submit Request.
  9. Once approved, you will receive a MyChart message notifying you that proxy access has been granted.

You can also grant an adult proxy access to your MyChart by completing the Adult Proxy Form below. To submit proxy forms, please send them via email to the Health Information Management Team at healthinformation@froedtert.com
